COPD is a progressive and heterogeneous condition, that may gradually lead to severe disability with very heavy clinical burden. The major risk factor for COPD is exposure to noxious gases and particles. In the western world, this means essentially being exposed to tobacco smoke, while in some developing countries (especially in Asia and Africa), this could mean living in a house where biomass fuel is used to cook or heat [1]. Although different lung function trajectories have been ascribed to different patients’ personal history and intrinsic characteristics, a progressive destruction of lung parenchyma and a remodelling of the airways are key components of COPD pathophysiology [2, 3].
